UWAGA: Zamawiającym informuje, iż z uwagi na pytania, które wpłynęły do postępowania oraz w związku z koniecznością przygotowania wyjaśnień i dokonania ewentualnych zmian w dokumentacji postępowania - przedłuża termin składania ofert do godz. 9:00 w dniu 10 maja 2024 r.
1. Przedmiotem zamówienia jest sukcesywna dostawa materiałów opatrunkowych na potrzeby Pogotowia Ratunkowego w Jeleniej Górze.
2. Przedmiot zamówienia obejmuje cztery Pakiety (odrębne części zamówienia):1) Pakiet I - Materiały opatrunkowe oraz zestawy zabiegowe;
2) Pakiet II - Sterylne zestawy specjalistyczne, p/oparzeniowe;
3) Pakiet III - Sterylne, specjalistyczne zestawy ratunkowe;
4) Pakiet IV - Gąbka hemostatyczna.
3. Szczegółowy opis przedmiotu zamówienia w tym minimalne wymagania dotyczące produktów, gwarancji, opis wymagań zamawiającego w zakresie realizacji i odbioru przedmiotu zamówienia - został zawarty:
1) w Formularzu asortymentowo-cenowym - Załącznik Nr 2 do Zapytania ofertowego (dotyczy wszystkich Pakietów);
2) w projekcie umowy - Załącznik Nr 3 do Zapytania ofertowego (dotyczy wszystkich pakietów).
4. Wykonawca zobowiązany jest podać w Formularzu asortymentowo-cenowym nazwę oraz producenta oferowanego produktu (dotyczy wszystkich Pakietów).
